Official Rules:

 (1) Tickets will be available from October 23, 2007 through January 17, 2008.

(2) Void where prohibited by law.

(3) Must be at least 18 years of age to participate.

(4) Winner need not be present to win. Winner will be officially notified by telephone within 1 day of the date of the drawing. In the event a prize is not claimed within 2 days of notification or a winner cannot be notified, an alternative winner will be selected.

 (5)  Proceeds of the Thirteenth Annual Super Bowl Sweeps benefit Bears Care. A donation of $30 per entry is suggested.

(6) Bears Care is a charitable organization that is exempt from federal income taxation under Section 501(c)(3) of the Internal Revenue Code, and contributions to it generally are deductible for federal income tax purposes. Consult your tax advisor for details.

(7) Participation is limited to10 entries per household.

(8) Odds of winning will depend on number of tickets sold.

(9) Drawing to be conducted at 5183 Harlem Road, Rockford, Winnebago County, Illinois.

(10) One winner will be drawn on January 17, 2008.  

(11) No purchase or contribution is necessary to enter the drawing. Please send a self-addressed, stamped envelope with completed entry form, include proof of address (copy of driver's license, etc.).  No discrimination, special treatment or disqualification will be made based upon whether or not a donation, contribution or purchase was made. Donations do not increase an entrant’s odds of winning.

(12) Winner is responsible for all federal, state and local taxes and fees.

(13) By entering, participant agrees to hold harmless and indemnify Bears Care and each of their respective parents, subsidiaries, affiliates, directors, owners, employees and agents against any and all liability, damages or causes or action (however named or described), with respect to or arising out of either (I) entrant’s participation; or (II) the receipt or use of the prizes awarded in the promotion.  

I agree that by participating in the Thirteenth Annual Super Bowl Sweeps, I am bound by the official rules and understand my name and picture may be used in any publicity related to the drawing.
